the biggest pitfall to avoid is just make sure you harm/disturb the roots as little as possible. I'd recommend maybe using a spoon to dig deep around each plant, and just lift it out media and all and transfer it into a new pot.
I tried that when they awhile ago with tweezers. Sadly, most of the smaller roots got tangled in the the larger plants which reached the bottom of the pot. The sand peat ratio doesn't let me just peel the plan away in clumps, and there are about to start to speed up growth again.
