It seems like heat damage or it got too dry to me. What have your conditions been like lately? If it is pests canola oil can be used as a non toxic pesticide.

How much water does it get, and what type of lights is it under? if you could give some details on your setup, soil, water, etc. that would be good.

Usually i'd not water it for weeks cause it would never go dry. Never changed soil probably perlite, sphag.
walmart great value distilles water. 1 red/blue led growlight 6:00am to 11:25.

Do i need to mix it with something?

How long have you had the plant? I'm going to guess it got too dry based on your description.

since 2016 April 22 or 21st.

I'd agree that it got too dry, especially if it happened to get really hot for a few hours. Watering on a weekly schedule is probably far too dry outside of a humid terrarium or greenhouse.

The growth point looks healthy, so the plant still has a good chance of recovering.
