Pictures of a lame trap

Is this normal? A new trap is coming in on my plant and before it's formed it's already dead. Like this:

Kind of sucks. Does anyone know what might be the problem?
 
What are the conditions? Almost looks like a burn...

yeah, that trap is dead o.o; mine do that every now and then - i think it has to do with tissue damage while the leaf is growing, which cuts of nutrients to the trap in its infancy - killing the trap, but developing the leaf.
 
mine does this too. its not really a problem for me. I just water it, leave it in the sun and new traps grow normally
 
This happened to me a lot when they were under artificial lighting, it hasn't happened in months since I put them outside.
 
I think it's too wet and might need to go outside in the sunlight.
 
The next trap looks happy though. It might just be a one off.
